Our practice was formed in 1987. Since then we have been serving Northwest Indiana, in both Lake and Porter County. With a firm focus and emphasis on prevention and treatment of diseases, we treat all ages from newborn to geriatric. Our Board Certified providers have trained at some of the most respected and trusted medical schools and institutions in the country and are able to treat a wide array of medical problems.

We have a strong commitment to preventative medicine and seek to promote the wellness of each and every patient. Our services include care for all stages of life; acute and chronic disease management, ​preventative health, pediatrics, womens health, wellness screenings and physicals, labs, diagnostics, and more. Family Practice
Dr. B. Chhabra is board certified in Family Practice from the American Academy of Family Physicians and a member of the academy. The focus of his practice has been on preventative care, early detection, and early intervention. As a physician for over 30 years, he is a strong patient advocate who values quality patient care.
Suzanne Meeks, N.P.
Family Nurse Practitioner
Suzanne has been with our team for over 13 years. She is a certified nurse practitioner with a focus in primary and family care.
Manjeet Geeta, M.D.
Family Practice
Dr. Manjeet Geeta is a family medicine physician. She has been with our team for over 14 years. She strongly believes in working as a team in collaboration, to provide the best care and treatment for the patient.
Katrina Kozub, N.P.
Family Nurse Practitioner
Katrina has been associated with our practice since 2010. She is a certified nurse practitioner with a focus in primary and family care.
Samantha Wojcik, N.P.
Family Nurse Practitioner
Samantha is new to our practice, starting August 2021. She is experienced working in an urgent care setting.
